A. SUMMARY STATEMENT:

Under the supervision of the Youth Outpatient Coordinator, the Counselor conducts outreach, intake, direct service, and case management services to youth and their families with the SPA 8 Adolescent Intervention and Treatment Recovery Program. 

B. DUTIES AND RESPONSIBILITIES:
1.  INTAKE AND ASSESSMENT SERVICES (20%):

  1. Screen calls and walk-in clients and determine appropriate services
  2. Complete all necessary screening and eligibility forms.

2. CONDUCT PREVENTION AND ADVOCACY SERVICES FOR PROJECT PARTICIPANTS (60%)

  1. Manage caseload as assigned.
  2. Conduct assessment and develop prevention and case management plan.
  3. Implement case management plan including individual and family sessions.
  4. Provide linkages and coordinate support services.
  5. Maintain files and records according to agency, county, state, and federal guidelines.
  6. Submit other reports as assigned.

3. OUTREACH AND COMMUNITY INVOLVEMENT (20%):

  1. Conduct outreach to youth and families, particularly those who are underserved or difficult to reach and are at risk of developing serious mental illness.
  2. Work with schools, shelters, other service providers, etc. to provide education on mental health prevention.
  3. Participate in community-based planning, as appropriate (e.g. SPA 8 Council Meeting)
  4. Perform other agency duties as assigned.

C. QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):

1.   REQUIREMENTS:

  1. Ability to work with high-risk adolescents, juvenile justice/court ordered or probation youth.
  2. Obtain a class B drivers license within four (4) months of date of hire.
  3. Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
  4. Valid California Drivers License with proof of Automobile Liability Insurance.
  5. At least three years of recovery or a drug free lifestyle as of the date of the application.
  6. Ability to work with people of diverse cultural, educational, social, and economic backgrounds.
  7. Demonstrate knowledge and skill in computer operations.
  8. Pass a physical examination and TB test within one (1) week of date of hire.
  9. Pass a criminal background check to work with children and adolescents.

2.   PREFERRED QUALIFICATIONS:

  1. Bilingual in written and verbal Spanish and English strongly preferred.
  2. Bachelor’s Degree in Social Work/related field, or Certification in Alcohol and Drug Abuse Counseling and two (2) years of youth service experience in mental health and/or substance abuse field.
