A. SUMMARY STATEMENT:
The CARE Assessor/Youth Treatment Counselor, under the supervision of the Adolescent Treatment Coordinator, conducts assessments, outreach, direct services, and case management services to youth and their families with the SPA 8 Adolescent Intervention and Treatment and Recovery Program.
B. DUTIES AND RESPONSIBILITIES:
- INTAKE AND ASSESSMENT SERVICES (60%):
- Screen calls and walk-in clients to determine appropriate CARE and AOD services.
- Complete all necessary screening and intake eligibility forms.
- Request CARE assessment vouchers and schedule appointments with various referral sources.
- Conduct comprehensive assessment of each participant to determine treatment voucher category and provider.
- Collect and input GPRA data and follow up.
- Complete monthly CARE billing from assessments and coordination of services.
- CONDUCT TREATMENT AND ADVOCACY SERVICES FOR PROJECT PARTICIPANTS (30%):
- Manage caseload as assigned.
- Conduct diagnostic assessment and develop treatment plan.
- Implement treatment plan to include: individual, group, and family counseling services.
- Coordinate support services.
- Maintain files and records according to agency, county, state, and federal guidelines.
- Submit other reports as assigned.
- OUTREACH AND COMMUNITY INVOLVEMENT (10%):
- Conduct outreach to youth and families, particularly those who are underserved or difficult to reach.
- Participate in community-based planning, as appropriate (eg. SPA 8 Council meetings).
- Engage youth in community organizing and leadership development activities.
- Perform other agency duties as assigned.
C. MINIMUM QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):
- REQUIREMENTS:
- Must have State Alcohol and Drug Counseling Certification approved by ADP.
- Four-year college education related to field or equivalent work experience related to field.
- Permanent resident or U.S. citizen; proof of work eligibility upon hiring.
- Valid California Drivers License with proof of Automobile Liability Insurance
- At least three years of recovery or a drug free lifestyle as of the date of the application.
- Ability to work with people of diverse cultural, educational, social, and economic backgrounds.
- Demonstrated knowledge and skill in computer operations.
- Complete a physical examination and TB test within one (1) week of date of hire.
- Pass a criminal background check to work with children and adolescents.
- OPTIONAL REQUIREMENTS OR PREFERRED QUALIFICATIONS:
- Obtain a class B drivers license within four (4) months of date of hire.
- PREFERRED QUALIFICATIONS:
- Bachelor’s Degree in Social Work/related field or Certification in Alcohol and Drug Abuse Counseling and two (2) years of youth treatment services.
- Ability to work effectively with people of diverse cultures and economic backgrounds.
- Spanish language spoken and/or written.
- Strong time management and organizational skills.
