A. SUMMARY STATEMENT:
The Case Manager, under the supervision of the Program Coordinator and Clinical Supervisor, will conduct outreach, case management services and provide resources to clients in clinic, home, school, hospital, or health care facilities within the SPA 8 Mental Health Services Program.
B. DUTIES AND RESPONSIBILITIES:
- SCREENING SERVICES (20%):
- Screen calls and walk in clients to determine appropriate services.
- Complete all necessary screening forms and eligibility process.
- Effectively interviews client and family needs across a broad spectrum of areas, recognizing professional boundaries and the need for consultation from other disciplines
- Determines client’s financial ability, assist them with Medi-Cal application and variety of Federal, State, and County assistance programs.
- DIRECT SERVICE PROVISION (65%):
- Manages caseload as assigned.
- Interviews, counsels and assists clients with social problems pertinent to their illness, treatment and recovery.
- Assesses and consults with family member and friends about home environment, family relationships, and personality traits pertinent to client’s medical, mental and recovery needs.
- Collaborates with Clinical Therapists and Psychiatrist in the development of individualized service plans for participants.
- Makes appropriate referrals and support services to various public and private health, mental health or other agencies to resolve client’s personal and social problems.
- Coordinates support services such as translation, transportation, arrange for home care and child-care services as needed.
- Conducts independent and group parenting education and substance abuse workshops for parents and co-occurring disorder clients.
- Under supervision, provides a minimum of 65% direct case management service hours to Agency clients.
- Serves as a liaison with schools, probation and court offices, and other institutions as appropriate.
- Receives close supervision from a Clinical Supervisor.
- Participates in case team conference meetings, shares social, medical and or mental health status of client’s with other health care disciplines.
- Participates in trainings, workshops, and conferences based on level of skills and professional development.
- Maintains files and records according to agency, county, state, and federal guidelines.
- Follows Agency requirements regarding accurate and timely case records, statistical, and other administrative procedures.
- OUTREACH AND COMMUNITY INVOLVEMENT (15%):
- Conducts outreach to youth and families, particularly those who are underserved or difficult to reach.
- Participates in community-based collaborative, as appropriate (eg. SPA 8 Council meetings).
- Engages youth in community organizing and leadership development activities.
- Performs other agency duties as assigned.
C. QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):
- REQUIREMENTS:
- Must possess a Bachelors Degree in Social Work or related field.
- Bilingual language(s) spoken and/or written: Spanish or Cambodian a must.
- Minimum one (1) year of experience working with children, youth, and families in the mental health field.
- Ability to organize and prepare documentation in a timely manner.
- Strong organizational, written, and communication skills.
- Permanent Residence or U.S. citizen; proof of work eligibility upon hiring.
- Valid California Drivers License with proof of Automobile Liability Insurance.
- At least three years of recovery or drug free lifestyle as of the date of the application.
- Must pass a class B Driver License within four months of hired.
- U.S. Citizen, permanent resident or proof of work eligibility in the United States.
- Complete a physical examination and TB test with one (1) week of date of hire.
- Pass a criminal background check to work with children and adolescents.
- PREFERRED QUALIFICATIONS:
- Ability to work effectively with people of diverse cultural, educational, social, and economic background.
- Demonstrate background in working with the dual-diagnosis population.
