A. SUMMARY STATEMENT:

The Counselor, under the supervision of the YFP Coordinator will conduct outreach, intake, assessment, and home-based case management services.

B. DUTIES AND RESPONSIBILITIES:

1. CASE MANAGEMENT AND SUPPORTIVE SERVICES (70%):

  1. Manage caseload as assigned.
  2. Facilitate home-based support services provision or linkage to activities per case plan.
  3. Serve as a liaison with probation and court offices, and other institutions as appropriate.
  4. Conduct education, diversion, and developmental activities for youth and families.
  5. Provide follow-up services with clients and with other service providers.
  6. Maintain documentation on each client and family activity/service.
  7. Participate in weekly case conference meetings.

2. FILE REVEW SYSTEM (10%):

  1. Submit a Monthly Caseload Summary to the unit Coordinator.
  2. Maintain accurate documentation of treatment and other services in accordance with Federal, State, County and AADAP guidelines.
  3. Submit other records such as Service Summary Forms, Group Recordings, Service Fee Records and others.
  4. Document all outreach services performed.
  5. Perform other agency duties as assigned.

3. PERFORM OTHER FUNCTIONS AS ASSIGNED (5%):

  1. Act as agency liaison and client advocate to referring agencies.
  2. Perform other job-related functions and assist other units as assigned.

C. QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):

1. REQUIREMENTS:

  1. Four-years of college education related to field or equivalent work experience related to field.
  2. Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
  3. Valid California Drivers License and current automobile liability insurance.
  4. Must have at least three (3) years of recovery or a drug free lifestyle as of the date of application.
  5. Must be able to work with people of diverse cultural, educational, social, and economic backgrounds.
  6. Demonstrated knowledge and skill in computer operations.
  7. Demonstrate case management experience.
  8. Ability to pass criminal background check to work with children and adolescents.

2. PREFERRED QUALIFICATIONS:

  1. Language(s) spoken and/or written: English and Spanish.
  2. Bachelor’s degree in Social Work or related field.
  3. Demonstrated background in outreach and client advocacy services.
