A. SUMMARY STATEMENT:

The Counselor, under the supervision of the Youth and Family Program Coordinator, conducts outreach, intake, direct service, and case management services to youth and their families.

B. DUTIES AND RESPONSIBILITIES:

1. 1. CONDUCT OUTREACH AND A REFERRAL NETWORK FOR PROJECT SERVICES (10%):

  1. Conduct outreach activities that targets youth and families in high risk situations, including incarcerated and probations status of youth.
  2. Evaluate calls and walk-in’s from potential clients and determine appropriateness for services.
  3. Participate in community-based planning efforts.

2. PLAN, DEVELOP, AND IMPLEMENT DIVERSION ACTIVITIES FOR YOUTH (30%):

  1. Implement plans for field trips, social and recreational activities, and educational outings for program participants.
  2. Conduct leadership development groups.
  3. Conduct community functions i.e. health fairs, forums etc.
  4. Provide transportation on as needed basis to and from events and meetings for program participants.

3. CASE MANAGEMENT AND DIRECT SERVICE PROVISION (60%):

  1. Manage caseload as assigned.
  2. Conduct diagnostic assessment and develop treatment plan.
  3. Implement treatment plan to include: individual, group, and family counseling services.
  4. Coordinate support services.
  5. Maintain files and records according to agency, county, state, and federal guidelines.
  6. Submit other reports as assigned.
  7. Manage 30 client caseload, maintain case files and records.
  8. Prepare all documentation on compliance with Federal, State, Local, and Agency guidelines.

C. QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):

1. REQUIREMENTS:

  1. Four-year college education related to field or equivalent work experience related to field.
  2. Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
  3. Valid California Drivers License and current automobile liability insurance.
  4. Must have at least three (3) years of recovery or a drug free lifestyle as of the date of application.
  5. Must be able to work with people of diverse cultural, educational, social, and economic backgrounds.
  6. Demonstrated knowledge and skill in computer operations.
  7. Demonstrated ability to engage clients in recovery process.
  8. Ability to pass criminal background check to work with children and adolescents.
  9. Must be able to certify as a Class B driver in 4 months.

2. PREFERRED QUALIFICATIONS:

  1. First Aid and CPR certification from a recognized agency.
  2. Structured at risk youth counseling experience to include facilitation of groups.
  3. Bilingual in Spanish.
