A. SUMMARY STATEMENT:

Under the supervision of the YFP Coordinator conduct outreach and a referral network for project services; participate in the planning and implementation of diversion activities; provide support to counselors during rap groups, individual counseling, family counseling, and court hearings for project participants; provide support services and data for project participants; input participant data in ISIS system.

B. DUTIES AND RESPONSIBILITIES:

1. CONDUCT OUTREACH, ADVOCACY AND A REFERRAL NETWORK FOR PROJECT SERVICES (20%):

  1. Contact service providers and school personnel in contracted areas and provide information regarding project services.
  2. Provide referrals for youth and families to services not encompassed within project.
  3. Communicate with sensitivity to clients from diverse language and cultural backgrounds.
  4. Evaluate calls and walk-in’s from potential clients and determine appropriateness for Services.
  5. Prepare all documentation on compliance with Federal, State, City and Agency guidelines.
  6. Provide support and advocacy services for program participants.
  7. Participate in community functions i.e. health fairs, forums etc.
  8. Assist documentation and data collection.

2. PARTICIPATE IN PLANNING AND IMPLEMENTATION OF DIVERSION ACTIVITIES (30%):

  1. Participate in field trips, social and recreational activities, and educational outings for program participants.
  2. Develop and maintain strategies for client referrals.
  3. Attend unit meetings, clinical supervision and other meetings as assigned.
  4. Assist with the facilitation of small and large group sessions.
  5. Support counselors in the development of activities.

3. CASE MANAGEMENT SUPPORT (50%):

  1. Assist in maintaining participant files by documenting and record activities, contracts, grades, attendance, groups and other activities.
  2. Assist in the planning and execution of parent and youth workshops
  3. Enter all program activities and services in ISIS web base system.
  4. Enter required ISIS documents and data into mainframe system:
    1. Enter client service units into mainframe system.
    2. Confirm ISIS entry enrollment, assessment and participation.
    3. Review ISIS system on a monthly basis to verify completion of client enrollment.
    4. Update any changes in client treatment status into mainframe system.
    5. Enter admission and discharge client data into mainframe system.
    6. Summarize client data reports from ISIS system.
    7. Make required corrections to client personal information and participation in the ISIS system.
  5. Provide written reports and corresponding as assigned.
  6. Tabulate and submit weekly client service production logs.
  7. Promptly retrieve requested data.
  8. Maintain filing and documentation system.
  9. Conduct other administrative duties as assigned.

C. QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):

1. REQUIREMENTS:

  1. Two years of college degree related to field or equivalent work experience in related field.
  2. Permanent resident or U. S. Citizen; proof of work eligibility upon hiring
  3. Have at least one-year (1) recovery or a drug free lifestyle as of the date of application.
  4. Must be able to work with people of diverse cultural, educational, social, and economic backgrounds.
  5. Valid California Drivers License and current automobile liability insurance policy.
  6. Ability to pass criminal background check to work with children and adolescents.
  7. Demonstrate knowledge and skill in computer operations.
